Ideally, the Direct Chronic Care Program model is structured to emphasize and prioritize the intrinsic power of the relationship between a client and his/her family practitioner to improve health outcomes and lower overall health care costs. The HRP-DCC contract fee structure can enable practitioners to spend more time with their clients.
 
As a member of HRP-DCC, you are automatically entitled to certain privileges and benefits, all while paying a low and affordable monthly rate. Perks of membership include 24/7 direct access to Dr. Lewis via email, texts, and virtual visits.
 
You will also enjoy extended visits that are never rushed—you will truly feel heard and have time to talk about your medical concerns. Same day and next day appointments  are always available.
 
Your membership includes up to 5 hours worth of visits for 12 months (from the time of purchase), with Dr. Lewis and Health Coaches. You will also receive help and advocacy from Dr. Lewis regarding any needed specialty care coordination.
 
Client fees cover extended visits, clinical,and consultative services, care coordination, and comprehensive care management.
Membershp includes 20% off labs, anytime.
We do not accept insurance or participate in government programs, relying solely on client fees.
After first purchase you will be billed 11 monthly installments of $60.00. 
You may cancel your membership at any time in writing at least 30 days in advance. If your membership had already been paid in advance for a full a year, a refund will be given at a prorated rate.
